Not sure what you mean by "ill", but a good caption might be, "MAYHEM: when a bad day decides to become an overachiever."
or something to do with the guy sucking his thumb.

When I found Despair.com a few years back, this was my favorite. I added a Halo Theme to fit the page.

Edit: I didn't know despair had a way to make posters. I just used photoshop.
